A call from your manager asking for a note can create unnecessary pressure when you are unwell. In many cases, you do not need one straight away. Understanding online sick note versus self certification helps you provide the right evidence, avoid paying for documentation you do not yet need, and focus on recovery.
For UK employees, the key distinction is usually the length of absence. Self certification covers the first seven calendar days of sickness absence. A fit note, often called a sick note, may be needed from day eight if your employer requires medical evidence. An online consultation can be a practical route to a doctor-led assessment when a fit note is appropriate.
What is self certification?
Self certification is your own statement that you have been unable to work because of illness or injury. It is intended for short absences of seven calendar days or fewer. Calendar days include weekends and bank holidays, not only the days you were due to work.
For example, if you become unwell on a Friday and remain off until the following Thursday, that is seven calendar days. If you are still unable to return on the Friday, your absence has reached eight calendar days and your employer may ask for a fit note.
Your employer may have a company form, an absence-reporting portal or a procedure for notifying a manager. Follow that process promptly, especially on the first day of absence. Self certification is not a clinical diagnosis, and it does not require you to obtain a note from a doctor simply to confirm a brief illness.
Employers cannot require a fit note to support statutory sick pay for the first seven calendar days. They may ask you to complete their usual self-certification process, but a medical certificate is not ordinarily needed at this stage.
Online sick note versus self certification: the practical difference
Self certification is based on your account of why you could not work during a short absence. A sick note, formally known as a fit note or Statement of Fitness for Work, is medical evidence provided after an assessment by an appropriate healthcare professional.
A fit note is not issued automatically because an employee requests one. The clinician must consider your symptoms, diagnosis where relevant, treatment, likely recovery and the functional effect of your health on your work. This is particularly relevant where your role involves safety-critical tasks, driving, lifting, operating machinery, night work or sustained concentration.
A doctor may state that you are “not fit for work”. Alternatively, they may say you “may be fit for work”, with recommendations such as a phased return, altered hours, amended duties or workplace adaptations. The latter does not mean you must return before you are ready. It gives you and your employer a basis for discussing whether suitable adjustments are available.
The difference matters because the documents serve different purposes. Self certification records a short absence. A fit note provides clinical opinion where the absence is longer or where medical context would assist discussions about a return to work.
When might you need an online sick note?
An online sick note may be suitable when you have been absent for more than seven calendar days, need a medical assessment without waiting for a routine appointment, or require a clear record of your current fitness for work. It can also be helpful if your condition is affecting the type of work you can safely do rather than preventing all work.
A remote appointment is not appropriate for every concern. Urgent symptoms such as severe chest pain, signs of stroke, serious breathing difficulty, heavy bleeding, collapse, severe allergic reaction or immediate risk to yourself or someone else require urgent NHS emergency care. A video or telephone consultation does not replace emergency assessment.
For non-emergency concerns, a clinician can often assess many common conditions remotely by discussing your symptoms, medical history, medication, work duties and recovery progress. They may issue a fit note where clinically justified, recommend treatment or self-care, advise an in-person examination, or direct you to a more suitable service.
An online consultation is an assessment, not a document-only transaction. Be prepared to explain when your symptoms began, how they affect your work, whether you have had similar episodes, and what support or adjustments your employer could offer.
Can a private doctor issue a fit note?
Yes. A private doctor can issue a fit note following an appropriate medical assessment. A fit note from a private clinician may be used as evidence for an employer, although workplace policies can vary. It is sensible to check your employer’s absence policy if you are unsure what they need.
The quality and legitimacy of the assessment matter more than whether the consultation took place online or in person. Look for a UK-based service delivered by a GMC-registered doctor, with clear contact details, a defined clinical process and transparent pricing. You should receive documentation that identifies the clinician and provides the relevant period and fitness-for-work advice.

What your employer can reasonably ask for
Your employer can ask you to follow a reasonable sickness-reporting procedure. That may include notifying a named person by a certain time, confirming the expected duration of absence and completing self certification when you return.
After seven calendar days, they can ask for medical evidence to support an ongoing absence. A fit note is the usual form of evidence. Your employer may also discuss the recommendations on a fit note, including possible adjustments that could support a safe return.
You do not normally need to disclose every private medical detail to your manager. A fit note focuses on fitness for work and functional information, not a full clinical record. If you have concerns about privacy, ask your employer who will receive the documentation and how it will be stored.

Choosing the right route for your absence
If you have been off for up to seven calendar days, self certification is usually the right starting point. Keep your employer informed and complete their form if they ask you to do so. Seeking clinical advice can still be sensible if you need treatment or are worried about your symptoms, but you may not need a fit note.
If your absence reaches day eight, or you need a doctor’s view on whether you can return with adjustments, arrange an assessment. An online appointment can save time where your symptoms can be safely assessed remotely. However, choose in-person care when examination, tests or urgent treatment may be needed.
Do not wait until the last moment if your current fit note is due to end and you remain unwell. Arranging a review before it expires gives the clinician time to assess progress and consider whether further time off, changed duties or a return to work is appropriate.
A simple example
Someone with a viral illness who is absent from Monday to Friday will generally self certify. Someone who develops a significant back problem, remains unable to work into a second week and cannot safely carry out manual duties may need a fit note after medical assessment.
The right option is not about obtaining the quickest piece of paper. It is about matching the evidence to the length of absence and ensuring that your health, your role and a safe return to work are properly considered.
